Before you can be considered for admittance to the unit, you must "pre-qualify" by telephoning the commanding officer listed on the "contact us" page.
The forms below are located at http://resources.seacadets.org/, Choose NSCC/NSLCC forms under Administration then Administration Forms (Adobe Acrobat Reader ® is required to open and fill out the forms - click here to get it if you don't already have it.). Fill out the required forms and submit them to the Commander or Executive Officer for processing. NSCADM 001 - CADET APPLICATION AND AGREEMENT
Used to record pertinent personal Cadet information as well as standard releases.
NSCADM 004 - PARENTAL AGREEMENT
Used to request assistance from Cadet parents with unit operations and support.
NSCADM 007 - CADET ENROLLMENT/REGISTRATION FORM
Used to submit enrollments, re-enrollments and transfers of NSCC and NLCC Cadets.
NSC-20 - REPORT OF MEDICAL HISTORY FORM
Used to obtain personal medical history on NSCC/NLCC Cadets.
NSC-21 - REPORT OF MEDICAL EXAMINATION FORM
Used to record personal medical examination information on NSCC/NLCC Cadets.
For Officers, Adult Leaders and Midshipmen:
Please note that all adult leaders will be subject to a full background check.
NSCADM 003 - OFFICER/INSTRUCTOR/MIDSHIPMAN APPLICATION
Used to record pertinent personal Officer/Instructor/Midshipman information as well as standard releases and privacy statement.
NSCADM 005 - OFFICER VOLUNTEER QUESTIONNAIRE
Used to obtain personal references of potential adult volunteers.
NSCADM 006 - ADULT LEADER ENROLLMENT/REGISTRATION FORM
Used to submit enrollments, re-enrollments and transfers of NSCC Officers, Instructors, and Midshipmen.
